Across the Lifespan

Via Services is proud to introduce the "Across the Lifespan" program at Via West Campus.  There are three parts to the program, which are described below.
 

 

HEALTH SELF-MANAGEMENT ACROSS THE LIFESPAN   

Health Self-Management Across the Lifespan is a program funded by the Valley Foundation to increase the health, well being, personal safety and independence of individuals who have developmental or physical disabilities. The program consists of a 6-day summer program at Via West Campus, from July 24-29, 2011, that combines education with traditional camp activities like dancing and swimming. Additionally, Valley Foundation has provided a one-time grant for those who participate in the program which will pay all but $160.00 for two 3-day follow-up weekend respites to be scheduled in the fall or early winter. There will also be intermittent phone or email contact with clients, parents and caregivers to provide support between sessions.

   

The program is designed to help clients with disabilities meet the following objectives:

• Describe health as physical, mental and social wellbeing, rather than the absence of disease
• Develop an online personal health plan that addresses individual health risk factors
• Distinguish health problems from non-health problems and seek medical/nursing attention when appropriate
• Identify environmental factors that can influence health status and take appropriate steps to modify the environment
• Demonstrate the knowledge and skills needed to self-administer prescription and over the counter medications and treatments
• Demonstrate the No-Go-Tell personal safety model designed to prevent abuse
• Develop the knowledge and skills to self-manage a chronic disease as independently as possible

At the end of the first session, clients will bring home:

• A t-shirt, towel, visor, water bottle and tote bag with the Across the Lifespan logo
• Written and pictorial materials describing the session activities
• Online access to a completed health risk assessment
• Written personal health plan for physician approval
• A customized medication organizing system
• Awards and photos highlighting achievements during the sessions

 

PHYSICAL ACTIVITY ACROSS THE LIFESPAN  

Physical Activity Across the Lifespan is a program funded by the Valley Foundation to increase the physical fitness, health, wellbeing and independence of individuals who have developmental or physical disabilities. The program consists of three sessions at Via West Campus: a 6-day summer program from June 3 through July 1, 2011 that combines education with traditional camp activities; a 3-day follow-up summer program from August 3-5, 2011 and a 3-day weekend program to be scheduled in the fall or early winter. There will also be intermittent phone or email contact with clients, parents and caregivers to provide support between sessions.

The program is designed to help clients with disabilities meet the following objectives:

• Demonstrate a physical fitness routine that expends 200-400 calories a day
• Select exercises that follow the 3-2-1 principle for 1 hour of exercise (30 minutes of cardiovascular exercise, 20 minutes of strength, and 10 minutes of flexibility)
• Select a variety of exercises to establish a pleasurable and sustainable daily physical fitness routine
• Master technology-based exercise equipment that can be used at home to develop and maintain an effective physical fitness program
• Enter data the exercise portion of MyNetDiary.com using iPads and laptops
• Develop a structured fitness program to be shared with the client’s physician(s) for approval

At the end of the first session, clients will bring home:

• A t-shirt, towel, visor, water bottle and tote bag with the Across the Lifespan logo
• Written and pictorial materials describing the session activities
• Online access to the Exercise section of MyNetDiary.com
• Personalized 3-2-1 Exercise Book
• Written exercise plan for physician approval
• Awards and photos highlighting achievements during the sessions
 

NUTRITION ACROSS THE LIFESPAN  

Nutrition Across the Lifespan is a program funded by the Valley Foundation to increase the health, nutrition, cooking skills and independence of individuals who have developmental or physical disabilities. The program consists of three sessions at Via West Campus: a 6-day summer program from July 17-21, 2011 that combines education with traditional camp activities; a 3-day follow-up summer program from August 17-19, 2011 and a 3-day weekend program to be scheduled in the fall or early winter. There will also be intermittent phone or email contact with clients, parents and caregivers to provide support between sessions.

The program is designed to help clients with disabilities meet the following objectives:

• Demonstrate use of My Pyramid and Nutrition Facts labels for menu planning
• Master the evidence-based Go-Slow-Whoa nutrition model to choose appropriate types and portions of healthy foods
• Demonstrate effective strategies for healthy meal planning and shopping
• Cook and prepare healthy snacks and meals from Go-Slow-Whoa cookbooks
• Select balanced meals during field trips to local restaurants
• Enter food intake data into MyNetDiary.com using iPads or laptops

At the end of the first session, clients will bring home:

• A t-shirt, towel, visor, water bottle, apron and tote bag with the Across the Lifespan logo
• Written and pictorial materials describing the session activities
• Online access to the Food section of MyNetDiary.com
• Colorful materials and recipe books demonstrating Go-Slow-Whoa meals and snacks
• Awards and photos highlighting achievements during the sessions
